Reward Card - Lost Card

If you have lost your reward card please fill in the short form below. If your card was registered or you can find the reward card number (at a bottom of a receipt) we will transfer any points earned to a new card and send it to the address provided below.

We will inform you by email if we are unable to transfer any points if the card is not registered or you are unable to provide a card number.

  • If you have a receipt where the lost card had been used, you can find the card number at the bottom of the receipt. It will begin with 900
  • Your telephone number will only be used if we have any issues registering your card
